Tütün bağımlılığında sitizin tedavisi: Tek merkezli kohort çalışması
Amaç: Bu çalışma, tek merkezli bir üniversite hastanesi sigara bırakma polikliniğinde sitizin tedavisi başlanan hastalarda kısa ve orta dönem sigara bırakma sonuçlarını, tedavi kesilmesini, relaps örüntüsünü ve öz-bildirime dayalı yan etkileri değerlendirmeyi amaçlamıştır.
Yöntem: Prospektif kohort tasarımındaki çalışmaya, 02.05.2024–31.12.2024 tarihleri arasında bir üniversite hastanesi sigara bırakma polikliniğinde sitizin başlanan hastalar dahil edildi. Hastalar 1., 3. ve 6. aylarda telefonla izlenerek sigara bırakma durumu değerlendirildi. Nikotin bağımlılık düzeyi Fagerström Nikotin Bağımlılık Testi (FNBT) ile ölçüldü. Sağkalım analizi Kaplan–Meier yöntemi ile yapıldı.
Bulgular: Çalışmaya dahil edilen ve altı ay süreyle takip edilen toplam 201 hastanın yaş ortalaması 42,38±15,01 yıl ve %83,1 erkekti. Hastaların %27,4’ünde tedavinin 5. gününde sigarayı bırakamama nedeniyle tedavi sonlandırıldı. Sigara bırakma oranları 5. günde %72,6; 1. ayda %43,3; 3. ayda %28,9 ve 6. ayda %21,4 olarak bulundu. Çok değişkenli lojistik regresyon analizinde kronik hastalık varlığı 6. ayda sigara bırakma başarısı ile bağımsız olarak ilişkili bulundu (OR=6,60; %95 GA: 1,49–29,17; p=0,013). Kaplan–Meier analizinde tedavi başarısızlıklarının özellikle erken dönemde yoğunlaştığı gözlendi. FNBT puanı ≥6 olan hastalarda sigarasız kalma süresi anlamlı derecede daha kısaydı (ortalama 71 gün vs. 95 gün; log-rank p=0,029).
Sonuç: Sitizin tedavisi kısa dönemde anlamlı bırakma başarısı sağlamakla birlikte relaps özellikle erken dönemde belirgindir. Yüksek nikotin bağımlılığı sigarasız kalma süresini kısaltmaktadır. Kronik hastalığı olan bireylerde bırakma başarısının daha yüksek olması, birinci basamakta kronik hastalık izlemleri sırasında sigara bırakma danışmanlığının önemli bir fırsat sunduğunu göstermektedir. Erken dönemde yoğun davranışsal destek, tedavi başarısını artırabilir.
